Abstract: Introduction: The Nursing Process (NP) is a fundamental tool for nurses' professional practice and should be guided by nursing theories. Among the theories that underpin nursing care, Dorothea Orem's Self-Care Theory stands out. It is based on the principle that individuals are responsible for their own care. However, in certain circumstances—such as intraoperatively, immediately and late postoperatively, or in rare osteometabolic diseases—this capacity is compromised, requiring professional support. Objective: To describe the application of the Nursing Process in the intraoperative, immediate, and late postoperative periods for individuals with rare osteometabolic diseases, according to Dorothea Orem. Methodology: This is a qualitative, descriptive case study conducted in a hospital in the interior of Bahia, focusing on Paget's Disease of Bone (PDB) and Osteogenesis Imperfecta (OI). The NANDA, NIC, and NOC taxonomies were used to support the development of the nursing plan. Results: The patient presented several nursing problems, including pain and elevated blood pressure during the intraoperative period, requiring a fully compensatory system. In the immediate postoperative period, she experienced anxiety and intense cold, requiring the fully compensatory and supportive-education systems. In the late period, due to bone fragility and bed rest, a partially compensatory system was adopted. Discussion: The patient presented limitations that required different nursing systems according to Orem's Self-Care Theory. In the intraoperative and immediate postoperative periods, the fully compensatory system was adopted due to total dependence. Subsequently, she transitioned to the partially compensatory system, performing some activities with assistance. Given her anxiety, the supportive-education system was used to encourage autonomy. The theory guided individualized and effective self-care, monitoring the patient's progress. Conclusion: The study highlighted the importance of technical knowledge combined with clinical sensitivity in planning care that respects the limits of self-care, promotes autonomy, and ensures dignity. It also reinforced the importance of integrating theory and practice in improving care in complex and vulnerable contexts.
