Abstract: Perinatal anxiety is a significant public health concern, substantially affecting maternal mental health during pregnancy and the postpartum period. Although perinatal depression has received greater attention in the literature, anxiety disorders are highly prevalent and may negatively impact several aspects of motherhood, including breastfeeding success and the development of the mother–infant bond. This study aimed to analyze the influence of perinatal anxiety on breastfeeding and the establishment of the maternal–infant bond. A narrative literature review was conducted based on scientific articles published in national and international databases. The findings indicate that anxiety symptoms are associated with lower rates of exclusive breastfeeding, early breastfeeding discontinuation, difficulties in mother–infant interaction, and impaired maternal responsiveness. Furthermore, anxiety may compromise maternal self-efficacy and increase feelings of insecurity regarding newborn care. It is concluded that the early recognition of perinatal anxiety and the implementation of psychological support strategies are essential to promote better maternal and infant outcomes, strengthening breastfeeding and the emotional bond during the early years of life.
