AMBULATÓRIO INTERDISCIPLINAR DE SEGUIMENTO EM TERAPIA INTENSIVA – HUOP
O material tem por objetivo descrever a estrutura e funcionamento de um Ambulatório Interdisciplinar de Seguimento em Terapia Intensiva de um Hospital Universitário do Oeste do Paraná. Trata-se de um projeto de extensão universitário, realizado por equipe assistencial hospitalar, residentes e acadêmicos: psicologia, enfermagem, medicina, fisioterapia e serviço social. Todos os pacientes internados na UTI são convidados a retornar três meses após a alta hospitalar para avaliação por equipe multiprofissional. Também são realizadas reavaliações em seis meses e um ano, por contato telefônico, para avaliar o estado geral de saúde e o impacto pós a admissão na UTI. Para tanto, é realizada consulta individualizada, com avaliações por meio de instrumentos validados e, quando necessário, o paciente é encaminhado para acompanhamento nos serviços de referência.

Abstract:
The material aims to describe the structure and functioning of an Interdisciplinary Outpatient Follow-up in Intensive Care of a University Hospital in the West of Paraná. This is a university extension and research project, conducted by hospital care team, residentes and academics: psychology, nursing, medicine, physiotherapy and social work. All patients admitted to the ICU are invited to return three months after hospital discharge for evaluation by a multidisciplinar team. Reassessments are also carried out in six months and one year, by telefone contact, in order to assess the general health status and the impact after admission to the ICU. For this, individual appointment are carried out, with assessments using validated instruments and When necessary, the patient is referred for follow-up at th referral services.
