Abstract: Introduction: Exclusive breastfeeding is recognized as one of the most important practices for promoting infant health, ensuring immunological protection, adequate development, and strengthening the emotional bond between mother and baby. Despite the widely documented benefits and recommendations from international organizations, exclusive breastfeeding rates remain below the ideal, influenced by cultural, social, economic, and psychological factors. Objective: To gather and discuss the main impacts of exclusive breastfeeding, as well as effective strategies for promoting this practice in the maternal-infant context. Methodology: This is a scoping review conducted according to the recommendations of the Joanna Briggs Institute (JBI) and guided by the PICo model (Population, Intervention, and Context). The SciELO, MEDLINE, LILACS, and CINAHL databases were consulted. Studies published between 2015 and 2024, in Portuguese, English, and Spanish, were included. Results: Exclusive breastfeeding (EBF) was found to provide nutritional, immunological, and emotional benefits to the baby, in addition to reducing maternal risks for breast cancer, ovarian cancer, and type 2 diabetes. However, cultural factors, such as traditional beliefs and myths, and social factors, such as early return to work, hinder its continuation. Psychosocial aspects, such as maternal anxiety and depression, also negatively influence the practice. The most effective strategies for its promotion include educational actions, emotional support, strengthening public policies, creating environments favorable to breastfeeding, and qualified action by health professionals. Conclusion: Exclusive breastfeeding is an essential practice for the promotion of maternal and child health, but it still faces significant challenges. Therefore, promoting breastfeeding requires a joint effort between families, health services, and society, recognizing it as a right and a necessity for the healthy development of the child.
