• Abstract: ABSTRACT: INTRODUCTION - Adherence to drug therapy is a determinant factor for treatment success, which is a challenge in relation to multiple factors when referring to the elderly population. PURPOSE - To verify the adherence to the drug treatment of the hypertensive elderly by the scientific production. METHODS - This is an integrative review of the literature. After data collection and crossing of the chosen descriptors, 594 articles were evidenced. The inclusion criteria were applied, selecting 126 articles, of which 40 were found in the Scielo database using the combination Hypertension and adherence to medication and 86 articles in LILACS using the association "Hypertension and elderly". After applying the exclusion criteria 17, they met the research objective. RESULTS: The thematic division of the contents covered by the articles was carried out: Profile of the hypertensive patients who adhered or not to the drug treatment; Factors that make adherence to drug treatment difficult. CONCLUSION: Based on this review, it was possible to highlight the need for health actions aimed at the early identification of complications and complications that may negatively impact adherence to the pharmacological treatment for hypertension, such as the presence of comorbidities.
