ADEQUAÇÃO DA TERAPIA NUTRICIONAL ENTERAL EM UMA UNIDADE DE TERAPIA INTENSIVA: VOLUME PRESCRITO X INFUNDIDO
Os pacientes internados na UTI são alvo de uma gama de alterações endócrino-metabólicas e imunológicas que contribuem para o aumento de suas necessidades nutricionais, o que torna necessário uma dietoterapia eficaz. Contudo, frequentemente o volume prescrito não é infundido efetivamente devido a circunstâncias da rotina hospitalar. Assim o trabalho tem o objetivo de identificar o percentual médio de volume infundido inadequado das dietas via sonda enteral em pacientes internados na UTI de um hospital terciário em um período de doze meses entre 2018 e 2019. Estudo retrospectivo e transversal, realizado por meio da coleta de dados secundários de indicadores de qualidade da unidade hospitalar que quantificam o percentual médio mensal do volume infundido inadequadamente e possíveis causas. A amostra contou com dados da dieta de pacientes recebendo terapia nutricional enteral exclusiva, excluindo dieta zero e outras vias de administração da dieta (via oral e parenteral). Conforme os resultados observou-se uma infusão do volume inadequada em 41,6% entre esses 12 meses, superior à meta (<20%), correspondendo a uma média de 68,6 dietas não administradas corretamente por mês. Entre os principais motivos destacam-se principalmente a ausência de relato pela equipe de enfermagem (39,2%), seguido de exames e procedimentos cirúrgicos (19,1%), extubação (8%), ocorrências na sonda (8%), diarreia (7,5%) e êmese/estase (9%). Diante deste cenário, é relevante o estabelecimento de protocolos e guias para padronização de condutas, bem como o treinamento permanente e continuado das equipes de assistência. Assim, é possível um maior preparo e conscientização sobre a importância da terapia nutricional, favorecendo uma política de assistência em saúde de qualidade, eficiente e eficaz.

Abstract:
Patients admitted to the ICU are the target of a range of endocrine-metabolic and immunological changes that contribute to the increase in their nutritional needs, which makes an effective diet therapy necessary. However, the prescribed volume is often not effectively infused due to routine hospital circumstances. Thus, the work aims to identify the average percentage of inadequate infused volume of diets via enteral tube feeding in patients admitted to the ICU of a tertiary hospital in a period of twelve months between 2018 and 2019. Retrospective and cross-sectional study, carried out through the collection secondary data from hospital unit quality indicators that quantify the average monthly percentage of the volume infused inappropriately and possible causes. The sample included data on the diet of patients receiving exclusive enteral nutritional therapy, excluding zero diet and other routes of diet administration (oral and parenteral). According to the results, an inadequate volume infusion was observed in 41.6% between these 12 months, higher than the target (<20%), corresponding to an average of 68.6 diets not properly administered per month. Among the main reasons, the absence of reports by the nursing team (39.2%), mainly followed by exams and surgical procedures (19.1%), extubation (8%), occurrences in the enteral tube feeding (8%), diarrhea (7.5%) and emesis / stasis (9%). In view of this scenario, it is relevant to establish protocols and guides for the standardization of conduct, as well as the permanent and continuous training of assistance teams. Thus, greater preparation and awareness of the importance of nutritional therapy is possible, favoring a quality, efficient and effective health care policy.
