Abstract: This clinical case intends to illustrate an interesting and rare clinical case, about a prostatic neoplasm with an atypical profile in nuclear medicine images, explaining basic concepts of the prostatic neoplasia propaedetics, and analyzing the particularities of this case. This regards a 75-year-old patient, with some comorbidities and in regular annual urological follow-up, asymptomatic, who had abnormal routine rectal examination and altered PSA (2.6 ng/ml). Prostate cancer was very likely, once the prostate MRI result was PIRADS 5, and the ultrasound-guided prostate biopsy was Gleason 10. However, surprisingly, systemic staging by PET-CT PSMA and FDG were negative. After surgical resection, the biopsy of the specimen showed that it represented a very rare subtype prostate adenocarcinoma with signet cells, and with a poor prognosis. Such a cell subtype neoplasm is most common in the stomach. This case is unusual due to the presence of this rare cell subtype, which exemplifies the rapid and aggressive evolution of the neoplasm, prompting the possibility that the PET CT PSMA and FDG were negative probably because of its high cell differentiation.
