ACOMPANHAMENTO FARMACOTERAPÊUTICO EM PACIENTES CADASTRADOS NA ESTRATÉGIA SAÚDE DA FAMÍLIA
A atenção farmacêutica é uma parte do componente profissional onde o farmacêutico interage de forma direta com o paciente identificando suas necessidades tanto farmacológicas quanto sociais para intervir quando necessário e proporcionar uma melhor qualidade de vida ao indivíduo. O objetivo desse trabalho foi realizar o acompanhar farmacoterapêutico em pacientes da Estratégia Saúde da Família com o intuito de identificar e solucionar possíveis interações medicamentosas e problemas relacionados a medicamentos. A metodologia utilizada para a realização do estudo consistiu na utilização do método Dader adaptado. Assim, identificou-se um total de 30 comorbidades tendo a hipertensão arterial e diabetes as mais incidentes. As possíveis interações medicamentosas encontradas foram interações fármaco/fármaco e interações fármaco/ alimento obtendo um percentual de 30,7% e 15,3% respectivamente e 54% dos pacientes não apresentaram nenhuma interação medicamentosa. Concluiu-se que a participação do farmacêutico é importante para diminuir as possíveis interações medicamentosas e consequentemente melhorar a qualidade de vida desses pacientes.

Abstract:
Pharmaceutical care is a part of the professional component where the pharmacist interacts directly with the patient, identifying their pharmacological and social needs to intervene when necessary and provide a better quality of life for the individual. The objective of this work was to carry out pharmacotherapeutic follow-up in patients of the Family Health Strategy in order to identify and solve possible drug interactions and drug-related problems. The methodology used to carry out the study consisted of using the adapted Dader method. Thus, a total of 30 comorbidities were identified, with arterial hypertension and diabetes the most incident. The possible drug interactions found were drug / drug interactions and drug / food interactions, obtaining a percentage of 30.7% and 15.3% respectively and 54% of the patients did not present any drug interactions. It was concluded that the participation of the pharmacist is important to reduce possible drug interactions and consequently improve the quality of life of these patients.
