ACOMPANHAMENTO FARMACOTERAPÊUTICO EM CONSULTÓRIO FARMACÊUTICO EM UMA UNIDADE BÁSICA DE SAÚDE DO MUNICÍPIO DE CANOAS
os serviços farmacêuticos na Atenção Primária podem contribuir para a diminuição do número de internações, à assistência aos pacientes com doenças crônicas e à prática de educação em saúde. A estruturação do funcionamento da Atenção Farmacêutica estabelece uma abordagem essencial para a promoção da saúde. O presente trabalho visou avaliar o impacto do acompanhamento farmacoterapêutico através do consultório farmacêutico em uma Unidade Básica de Saúde do município de Canoas/RS. Os pacientes foram recebidos mediante encaminhamento dos médicos. O período de estudo foi de dois meses, sendo realizadas quatro consultas com cada paciente. Aplicou-se o Formulário Padronizado para Realização da Consulta Farmacêutica e realizou-se Estratificação de Risco Cardiovascular para os Hipertensos. Foram realizadas intervenções a partir das necessidades de cada paciente e retorno para avaliação. Participaram do estudo 8 pacientes com média de idade de 69 anos. A partir do acompanhamento os participantes do estudo que apresentaram IMC >24,9 foram encaminhados ao Programa Canoas Mais Leve. Os pacientes relataram estar realizando caminhadas e observaram melhora na disposição física. Os diabéticos usuários de insulina relataram estar armazenando corretamente a mesma, estar alternando local de aplicação, e mudança na forma de homogeneizá-la. Apontaram melhora na hidratação dos pés e no entendimento do autocuidado. Identificou-se três participantes acometidos por duplicidade terapêutica e quatro com alto risco cardiovascular os quais receberam encaminhamento aos cuidados do médico. Conclui-se que a implementação do Cuidado Farmacêutico na rede de saúde contribui para promoção do uso racional de medicamentos, para o fortalecimento da APS e integração à rede de saúde com a finalidade de otimizar e qualificar o acesso aos medicamentos.

Abstract:
the pharmaceutical services on the Primary Health Care may contribute to the decrease of the hospitalization numbers, they may also contribute with the assistance of the patients with chronic conditions and with the health education practices. The functioning structure of Pharmaceutical Care establishes an essential approach to health promotion. This paper aimed to access the impact of the pharmaceutical monitoring in the pharmaceutical office on a Basic Health Unit of the city of Canoas/RS. The patients were received through medical referrals. The study period was of two months, with four appointments with each one of the patients. The Standardized Form of Pharmaceutical Consultation was applied and Cardiovascular Risk Stratification was conducted in the cases of the hypertensives. Interventions were conducted accordingly to each of the patients’ necessities as well as the coming back appointment for an assessment. 8 patients in the average age of 69 years old were part of the study. Following the monitoring of the participants of the study who presented BMI >24,9 were referred to the Canoas Mais Leve program. The patients reported that they were practicing hiking and that they had observed a physical arrangement increase. The insulin users diabetic patients reported that they were storing it correctly and that they were also changing the application site and the way of homogenize it. They pointed out an improvement on the feet hydration and in the understanding of self-care. There were identified three participants affected for therapeutic duplicity and four of them with a high-level cardiovascular risk that were referred to the medical care. It is possible to conclude that the implementation of the Pharmaceutical Care on the health network contributes to the promotion of the rational usage of medicines, to the reinforcement of the APS, and integration to the health network aiming to optimize and qualify the access to the medicines.
