Abstract: Introduction: In Brazil, the demographic transition has resulted in a growing demand for specific health services for the elderly population, which presents a morbidity profile predominantly marked by chronic diseases and conditions that require continuous and integrated care. Objective: to analyze through literature the challenges faced by the Unified Health System (SUS) in guaranteeing access and equity in comprehensive health care for the elderly population. Methodology: This is a descriptive Integrative Review, based on a critical, informative and reflective analysis of access and equity for elderly health, with studies analyzed from 2019 to 2023 on the topic in question. Data collection was carried out in August 2024, in the LILACS and SCIELO and PUBMED databases. Results: These findings indicate that elderly people may be facing significant barriers in accessing adequate, integrated and equitable care, highlighting failures in PHC to guarantee quality support that meets their specific health needs. Conclusion: Strengthening the role of the State and the development of specific public policies for dependent elderly people and their caregivers are essential to ensure support, protection and well-being for this vulnerable population.
