Abstract: Introduction. Tropical pyomyositis is an acute infectious entity of the skeletal muscles, with a high potential for chronicity, with the potential to affect the individual systemically. Physiotherapy intervenes from the pre-operative period until hospital discharge, promoting functional return and resumption of the functional milestones of sitting, standing and walking. Objective. To analyze the application of a physiotherapeutic protocol in the pre and postoperative period of osteomyelitis in the lower limb associated with tropical pyomyositis in a reference hospital in Recife. Method. A quantitative, before-and-after study was carried out, the sample of which was composed of individuals of both sexes, admitted to the trauma ward of Hospital Otávio de Freitas. Among the eligibility criteria, they were included because they presented a clinical and/or complementary diagnosis of tropical pyomyositis and osteomyelitis in the lower limb confirmed through imaging examination, and were excluded because they presented a cognitive deficit identified through the mini-mental state examination, which did not allow them to follow the requested commands, refracture in the same segment or undergo some type of adjuvant treatment, except physiotherapy. He underwent a physiotherapeutic protocol in the pre- and post-hospital phases. Results. Individual, female, 11 years old, 26kg, 1.49m tall, was diagnosed with tropical pyomyositis associated with multiple abscesses, progressing to chronic osteomyelitis in the distal femur. After a surgical approach, physiotherapeutic interventions were performed to control pain, regression of edema and recovery of mobility and muscle strength, as well as resumption of functional transfer milestones. Conclusion. Physiotherapy intervenes from the pre-operative period until hospital discharge, promoting functional return and resumption of the functional milestones of sitting, standing and walking. The absence of validated protocols as well as the scarcity of literature seem to be a good starting point for new research.
