Abstract: The treatment of thoracic aortic aneurysms (TAA) has evolved significantly, with open surgery and endovascular repair (TEVAR) emerging as the main therapeutic approaches. This integrative review compared these two modalities, evaluating their respective advantages and limitations in terms of mortality, morbidity, recovery time, quality of life and durability of results. TEVAR demonstrated lower perioperative mortality and morbidity, as well as a shorter recovery time, making it a preferred option for high-risk patients. However, the inferior durability of TEVAR, associated with a greater need for reinterventions, highlights open surgery as a more appropriate choice for young patients or those with complex aneurysm anatomy. The choice between TEVAR and open surgery should be individualized, considering anatomical and clinical factors and life expectancy of patients. Future studies focused on technological innovations and long-term follow-ups are needed to improve therapeutic strategies and optimize outcomes in patients with TAA.
