A ULTRASSONOGRAFIA CINESIOLÓGICA: ATUAÇÃO NO DESMAME E DIAGNÓSTICO DE DISFUNÇÃO DIAFRAGMÁTICA NA UTI

• Abstract: Introduction: Kinesiological Ultrasound (US) has gained notoriety for its morphological observation of muscle structures in real time. It is considered a valid imaging test for predicting ventilator weaning failure and possible diagnosis of Diaphragmatic Dysfunction (DD). Objective: Describe the effectiveness of USC as a parameter of weaning success and a predictor of extubation failure, associating the tool with the diagnosis of DD and bedside respiratory rehabilitation. Methods: An integrative and descriptive review, using the PRISMA model for indexed databases, was conducted through extraction in PubMed, ScienceDirect and Cochrane Library, published in January 2018 to February 2024, without restriction to language with the possible descriptors in association with the Boolean operation and: Ultrasonography, Extubation, Diaphragm ultrasound, Early rehabilitation, Diaphragm dysfunction, Mechanical Ventilation, Intensive Care Units. Results: 11 articles were included, about 8 of which showed that SBU is a useful tool in weaning and predicting extubation failure, while the other articles proved its effectiveness in detecting CD through sensitivity and diaphragmatic thickening. Patient rehabilitation considered Inspiratory Muscle Training, bedside physical mobilization and protective mechanical ventilation to be the main methods for improving the clinical condition of ICU patients. Conclusion: USC is considered an efficient tool for predicting extubation success and ventilator weaning failure. Although it is not considered the only way to screen for diaphragmatic dysfunction, it is considered promising for monitoring the diaphragm muscle.
