Abstract: Obesity is one of the main public health epidemics of the 21st century, characterized by the excessive accumulation of body fat and an increased risk of chronic non-communicable diseases such as type 2 diabetes mellitus, hypertension and cardiovascular diseases. Treatments for obesity vary between drug and non-drug approaches, with physical exercise being an important option due to its beneficial role in the rehabilitation of various diseases. Physical exercise can be classified into aerobic and resistance, with the latter focusing on muscle strengthening and improving body composition. This study systematically reviews the effects of resistance exercise on obese adults, highlighting its physiological and metabolic influences. The results showed that resistance exercise promotes improvements in body composition, such as an increase in muscle mass and a reduction in fat mass, as well as contributing to the preservation of bone mass. An important effect was improved insulin sensitivity, which is crucial in controlling metabolic diseases such as diabetes. Resistance exercise also regulates metabolic biomarkers, such as a reduction in leptin and an increase in adiponectin, both of which are related to a reduction in chronic inflammation and the control of cardiovascular diseases. Combining this type of exercise with appropriate nutritional strategies can enhance these benefits. It is concluded that resistance exercise is an effective approach to managing obesity in adults, providing not only an improvement in body composition, but also acting as a protective factor against diseases associated with obesity. It should therefore be included as part of therapeutic strategies, together with nutritional monitoring and, when necessary, the use of medication.
