Abstract: Systemic Arterial Hypertension (SAH) is a pathology that is often asymptomatic, silent, and is identified by measuring blood pressure, which may present persistent blood pressure values above normal. In this sense, it is important to highlight that the elderly are the most susceptible to the disease. Furthermore, the presence of other pathologies leads to the practice of polypharmacy, resulting in the emergence of Medication-Related Problems (DRPs), making treatment adherence and effectiveness difficult. Thus, the pharmaceutical professional assumes real importance in providing guidance and clarification regarding the medications to be administered, as well as their adverse events. The research aims to demonstrate the importance of the pharmaceutical professional in pharmacotherapeutic monitoring in elderly hypertensive patients. Therefore, this research is a literature review based on scientific articles indexed in the following health databases: Scielo, Lilacs and PubMed. A time frame was made that covered the last six years, more precisely the period between 2010 and 2023. Upon reading the selected scientific journals, three categories emerged: the importance of pharmaceutical monitoring to reduce adverse events; difficulties in reducing the use of polypharmaceuticals in the elderly; and the role of the pharmacist in promoting the quality of life of hypertensive elderly people. This research highlighted the importance of pharmaceutical professionals in monitoring elderly hypertensive patients, as well as highlighting the role of this professional in reducing side effects resulting from the use of polypharmaceuticals. The main pharmaceutical interventions for the quality of life of hypertensive elderly people, who use multiple drugs, were also addressed.
