Abstract: Maternal and congenital syphilis remain major public health challenges in Brazil and worldwide. Vertical transmission, if not prevented, results in severe outcomes such as miscarriage, stillbirth, prematurity, and congenital malformations. Benzathine penicillin is the recognized treatment of choice, safe and effective in preventing disease transmission from mother to fetus. However, many health professionals still resist administering this drug in primary health care settings due to fear of adverse effects, particularly anaphylaxis. This chapter analyzes the importance of benzathine penicillin administration during prenatal care, the barriers faced by health professionals, and the advances brought by local and national protocols, while incorporating epidemiological evidence and practical recommendations from primary health care educational materials. The discussion highlights the need to overcome institutional and individual barriers, invest in continuous training, and ensure support for health teams, promoting greater resoluteness in primary care and improved maternal and child health indicators.
