Abstract: Objective: To describe and discuss the adherence of pregnant women up to 12 weeks of gestation to prenatal care by the Unified Health System in Macaé / RJ, comparing it to the State of Rio de Janeiro and Brazil. Methodology: This is an epidemiological, descriptive, retrospective research with a quantitative approach. To carry out the data collection, the research resorted to official and publicly available secondary databases on the health care of women during the puerperal pregnancy period. The data collected refer to the period between the years 2016 and 2021 (1st semester). Results: There was an increase in the number of pregnant women with the first prenatal care up to the 12th week of gestation, going from a percentage of 18% in 2017 to 44% in 2021, showing a growing increase in the number of pregnant women with prenatal care. -early birth ranging from 34 pregnant women (18%) in 2017 to 466 pregnant women (44%) in 2021; with the exception of 2020, which showed a small reduction with 248 pregnant women (34%). For pregnant women with a late start to prenatal care, there was a decrease ranging from 155 women (82%) in 2017 to 784 pregnant women (56%) in 2021. Final Considerations. A growing municipal dynamic in expanding uptake was revealed precocious. The municipality tripled the percentage of pregnant women treated while the state and national average continued to grow around doubling the number of pregnant women treated early.
