Abstract: This article analyzes the importance of home visits in the follow-up of older adults with comorbidities, based on recent scientific evidence. The study explores how this practice improves therapeutic adherence, reduces hospitalizations, strengthens the bond between healthcare teams and patients, and promotes equity in primary care access. A comparative analysis of 25 studies revealed that the presence of health professionals at home enhances the clinical and emotional effectiveness of interventions, especially when combined with digital tools. The research also highlights the key role of home visits in supporting family caregivers, providing health education and psychosocial assistance. The conclusion emphasizes that home visits should be recognized as a central strategy in elderly care, essential for a more humanized, continuous, and integrated approach to managing chronic conditions in aging populations.
