Abstract: Tolosa-Hunt Syndrome (THS) is a rare inflammatory condition affecting the cavernous sinus or superior orbital fissure, causing severe retro-orbital pain and ocular paralysis. In the reported case, a 36-year-old female presented with frontotemporal headache, nausea, and subsequent bilateral ophthalmologic symptoms, including reduced visual acuity, photophobia, diplopia, and strabismus. Laboratory tests and imaging studies (MRI and angiography) showed no significant findings. Based on the hypothesis of THS, corticosteroid therapy with Prednisolone 60 mg/day was initiated, leading to rapid and complete symptom resolution. THS was diagnosed by exclusion, based on the dramatic response to treatment and the absence of other causes such as tumors or vascular pathologies. This case highlights the importance of early diagnosis and a multidisciplinary approach, especially in rare bilateral presentations, to prevent complications and ensure effective treatment.
