Abstract: Food Protein-Induced Enterocolitis Syndrome (FPIES) in adults remains a poorly recognized and frequently underdiagnosed condition, compromising adequate clinical management. This study aims to review and synthesize current evidence regarding the pathophysiology, clinical manifestations, diagnostic criteria, and therapeutic strategies of adult FPIES, seeking to improve recognition of the syndrome in clinical practice. A narrative literature review was conducted based on clinical studies, reviews, consensus statements, and guidelines published between 2016 and 2026, focusing on the particularities of adult disease presentation. Findings indicate that FPIES is predominantly mediated by immunoglobulin E-independent immune responses and is mainly characterized by gastrointestinal symptoms, including vomiting, diarrhea, and abdominal pain, which are often mistaken for other conditions. The absence of specific laboratory tests highlights the importance of detailed clinical history for diagnosis. Therapeutic management relies on identifying and eliminating the triggering food, combined with patient education to prevent recurrence. Adult FPIES presents clinical characteristics distinct from the pediatric form, requiring greater awareness among healthcare professionals and further studies to clarify its pathophysiology and clinical evolution, aiming to reduce underdiagnosis and improve patient care.
