Abstract: Telemedicine has emerged as a strategic tool to optimise access, quality and continuity of paediatric care, especially in geographical or social contexts where structural barriers persist. Its implementation has accelerated in the last decade, most notably during the COVID-19 pandemic, when it enabled the maintenance of medical care for children and adolescents through remote consultations, chronic disease management and mental health support. This study consists of a narrative literature review, based on indexed scientific sources and documents from international organisations, using DeCS/MeSH descriptors related to telemedicine, paediatrics and health equity. Both opportunities and challenges related to this modality were identified. Benefits include reduced waiting times, access to specialists in rural areas, continuous monitoring of chronic diseases and health education for caregivers. However, challenges remain, such as the digital divide, limitations in physical examination, cultural barriers and the absence of adequate regulation. The discussion highlights successful experiences in countries such as the United States and the United Kingdom, in contrast to the structural limitations observed in Latin America. In conclusion, paediatric telemedicine represents an innovative and complementary avenue of care, the success of which will depend on its equitable and sustainable integration into health systems.
