Abstract: Objective: To systematically review current evidence on psoriasis in children under 10 years, addressing clinical features and therapeutic options with focus on safety and efficacy. Methods: A systematic review following PRISMA 2020 was conducted using PubMed, Scopus and SciELO databases (2019–2025). From 862 records identified, 28 were included. Results: Pediatric psoriasis affects 0.1–1% of the population, mainly between ages 6–10. Plaque and guttate forms predominate. Topical corticosteroids and vitamin D analogues are first-line treatments; NB-UVB phototherapy or systemic agents (methotrexate, cyclosporine) are used for moderate-to-severe disease. Biologics (etanercept, ustekinumab, secukinumab, ixekizumab) achieved mean PASI 75 of 82% and PASI 90 of 68%, with <3% serious adverse events. Conclusion: Management should focus on topical and phototherapy modalities, reserving systemic or biologic agents for severe or refractory cases. Individualization by age, weight and psychosocial context is essential.
