Abstract: Cervical cancer remains one of the most prevalent gynecological malignancies affecting women of reproductive age, with significant repercussions on fertility and quality of life. Fertility preservation has become increasingly relevant due to advances in conservative surgical techniques, such as radical trachelectomy, and assisted reproductive technologies, including cryopreservation of oocytes, embryos, and ovarian tissue. Additional approaches, such as neoadjuvant chemotherapy combined with conservative surgery and ovarian transposition, have expanded reproductive options for patients. Nevertheless, these strategies present challenges, such as higher obstetric risks, altered uterine immunological responses, and limited access to specialized care. Thus, a multidisciplinary approach that integrates oncology, gynecology, reproductive medicine, and psychological support is essential to ensure favorable oncological outcomes while safeguarding reproductive potential.
