Abstract: The accelerated aging of the Brazilian population has assigned Long-Term Care Institutions for Older Adults (ILPIs) a structural role within the contemporary health-care model. Beyond providing shelter, these institutions engage in health promotion, comprehensive care, and the safeguarding of dignity for individuals who have lost autonomy or lack family support. Their historical trajectory demonstrates a significant transition: from religious philanthropic entities established during the colonial period, they have evolved into multiprofessional spaces governed by public policies and social rights. The clinical profile of residents, characterized by multimorbidity and high therapeutic complexity, makes polypharmacy a critical concern. The use of multiple medications increases the risk of adverse events, drug interactions, hospitalizations, and functional decline, all intensified by physiological changes inherent to aging. In this context, the pharmacist is essential in reviewing pharmacotherapy, identifying inappropriate medications, establishing safe protocols, and conducting deprescribing, thereby promoting greater therapeutic safety and improved resource management. Thus, the permanent integration of this professional into ILPIs is fundamental to ensuring safer, more efficient, and more dignified aging.
