POLICIZATION OF THE THIRD RAY FOR HAND AND FOREARM IN MUTILATING INJURIES OF THE THUMB: CASE REPORT

INTRODUCTION/FUNDAMENTALS - The study of politicization through the case report is of paramount importance for understanding this surgical technique for patients who presented mutilating lesions on the thumb. Knowledge of its post-surgical repercussions allows it to be addressed more often in thumb amputation victims. 

 

GOALS- To describe the possibility of transferring the third ray to the thumb in cases of mutilating lesions, resulting in a significant improvement in hand functionality.

 

DESIGN/METHODS - We sought to follow up on the described patient, with a complete anamnesis, review of medical records and current literature, using databases accessed via Pubmed and Scielo, seeking described articles related to policization, hand, forearm and transplant. Eighteen articles were obtained from 2004 to 2016 and, of these, 10 were selected that responded satisfactorily to the objectives of this work. Priority was given to the most current articles with high theoretical value.

 

RESULTS- I.L.S., male, 16 years old, Brazilian, single, evangelical, machinist (on leave), born in Barueri-SP and coming from Ibitinga-SP, suffered a mutilating lesion on the thumb and index finger of the right hand in a fiber milling machine. We opted for transposition of the metacarpal from the third finger to the thumb, with osteosynthesis using Kirschner wires. The patient showed great functional gain, presenting a pinching movement.

 

CONCLUSIONS/FINAL CONSIDERATIONS - With better knowledge about the technique and benefit of policing for patients with thumb amputation, a more effective approach is possible, in order to improve the quality of life of patients compromised by significant loss of hand functionality due to trauma.
