Abstract: The objective of this study was to describe the clinical and epidemiological profile of participants in community outreach activities conducted in Ouro Preto (MG), Brazil, estimate the prevalence of Systemic Arterial Hypertension (SAH) and Diabetes Mellitus (DM), and analyze factors associated with blood pressure control and the coexistence of these conditions. This was an observational, cross-sectional, and analytical study carried out in 2025, including 576 adult volunteers. Sociodemographic data, medical history, medication use, family history, blood pressure (BP), body mass index (BMI), and waist circumference (WC) were collected according to current guidelines. The prevalence of SAH was 32.6% and DM 10.1%. Among hypertensive individuals, 77.7% were above the therapeutic target, and 74.3% of those who reported controlled BP presented inadequate levels at measurement. Among participants who declared not having hypertension, 19.3% had BP values compatible with SAH, indicating underdiagnosis. Excess weight was identified in 60.7% of the sample and was present in 70.7% of hypertensive and 72.4% of diabetic individuals. High-risk WC was more frequent in women (66.2%) than in men (31.6%) and was highly prevalent among hypertensive (72.3%) and diabetic (65.5%) participants. A higher concentration of SAH and DM was observed in individuals over 50 years of age, as well as greater prevalence and poorer BP control among Black and Brown participants. These findings reinforce the importance of community-based screening strategies, health education, and strengthening primary care for the prevention, early diagnosis, and improved control of chronic non-communicable diseases.
