Abstract: Gestational diabetes mellitus (GDM) is a metabolic condition characterized by glucose intolerance, diagnosed for the first time during pregnancy. This condition carries significant risks for both the mother and the baby. Given the increasing prevalence of GDM and the need for safe and effective therapeutic strategies, physical exercise, especially aerobic exercise, has emerged as an important non-pharmacological intervention in prenatal care. In this sense, the objective of this study was to analyze the effects of regular aerobic exercise on glycemic control in pregnant women with gestational diabetes mellitus in the third trimester of pregnancy, through a review of the scientific literature. This is a descriptive review, with a qualitative approach, which searched for articles published between 2016 and 2026 in the PubMed and Google Scholar databases. After applying the inclusion and exclusion criteria, 14 studies were selected for further analysis. The results showed that regular moderate-intensity aerobic exercise, such as walking, swimming, and water activities, is associated with reduced fasting and postprandial glucose levels, as well as improved insulin sensitivity and reduced excessive weight gain during pregnancy. Studies have also indicated a reduction in maternal and fetal complications, as well as an improvement in the physical and emotional well-being of pregnant women. It can be concluded that aerobic exercise, when performed in a planned, supervised manner and adapted to individual conditions, is an effective, safe, and low-cost strategy for managing GDM, highlighting the importance of its inclusion in clinical practice and public health initiatives focused on prenatal care.
