Abstract: Introduction: Chronic rhinosinusitis (CRS) is a multifactorial disease that compromises quality of life and is often associated with respiratory comorbidities. Understanding its mechanisms is crucial for guiding effective therapeutic approaches. Methodology: A narrative review was conducted based on the Brazilian Consensus on Rhinosinusitis and studies indexed in PubMed and Embase. Articles from the last five years addressing the diagnosis, management, and clinical impact of CRS were included, followed by a qualitative analysis of the extracted data. Discussion: The treatment of CRS should follow a stepwise approach, starting with nasal irrigation and topical corticosteroids. Refractory cases may require functional endoscopic surgery, while biological therapies are emerging as an effective option in patients with type 2 inflammation. The individualization of treatment, guided by biomarkers and inflammatory profiles, reflects the advance toward personalized medicine. Conclusion: CRS requires continuous and multidimensional management, combining clinical, surgical, and biological strategies according to the patient's profile. The current focus is on quality of life and the selection of appropriate therapies. Investing in expanded access and biomarker research is crucial for enhancing therapeutic outcomes.
