MANAGEMENT OF GASTRIC CANCER IN PREGNANT WOMEN
RESUMO: INTRODUÇÃO: Neoplasias envolvendo o trato gástrico durante o período da gravidez em pacientes jovens, são em suma, bastante incomuns e as informações que podem serem colhidas sobre o tema se apresentam, no geral, em estudo de grandes populações sobre doenças malignas, não demonstrando, porém, os caso concretos, deste modo demonstrar a forma de apresentação e o manejo escolhido para o caso. RELATO DE CASO: Paciente de 26 anos, relatava que sentia há um ano dor em queimação em região epigástrica. Fora solicitada uma Endoscopia digestiva alta onde se encontrou uma lesão gástrica ulcero-infiltrativa de 10 cm na região da pequena curvatura do piloro indo até a incisura angular, colhida a biópsia do local foi enviada ao exame anatomopatológico sendo diagnosticado como carcinoma gástrico de células em Anel de Sinete ulcero-infiltrativo. Paciente na 25ª semana de gestação foi submetida a Gastrectomia subtotal, linfadenectomia e reconstrução em Y de Roux, tendo boa evolução no pós-operatório e sem complicações obstétricas. Parto realizado com 37 semanas e 6 dias de gestação, feto vivo do sexo masculino sem intercorrências. CONCLUSÃO: Assim, essa apresentação complexa e incomum do câncer gástrico e a gestação enfatizam a importância do diagnóstico precoce, a fim de garantir a melhor conduta, escolha terapêutica e prognóstico que estão diretamente relacionados ao estágio neoplásico e obstétrico.

Abstract:
ABSTRACT: INTRODUCTION: Neoplasms involving the gastric tract during pregnancy in young patients are, in short, quite uncommon and the information that can be collected on the subject is presented, in general, in a study of large populations on malignant diseases, not but demonstrating the concrete cases, thus demonstrating the form of presentation and the handling chosen for the case. CASE REPORT: A 26-year-old patient reported feeling burning pain in the epigastric region for a year. An upper digestive endoscopy was requested, where an ulcerative-infiltrative gastric lesion measuring 10 cm was found in the region of the lesser curvature of the pylorus going up to the angular notch, a biopsy was taken from the site and sent for anatomopathological examination being diagnosed as ring cell gastric carcinoma of ulcero-infiltrative signet. Patient in the 25th week of pregnancy underwent subtotal gastrectomy, lymphadenectomy and Roux-en-Y reconstruction, with good postoperative evolution and no obstetric complications. Delivery performed at 37 weeks and 6 days of gestation, live male fetus without complications. CONCLUSION: Thus, this complex and unusual presentation of gastric cancer and pregnancy emphasize the importance of early diagnosis in order to ensure the best management, therapeutic choice and prognosis that are directly related to the neoplastic and obstetric stage.
