Abstract: Introduction: The development of nursing care for elderly patients with heart failure (HF) during the transition to the home is vital for promoting self-care. This pathology predominantly affects individuals over 65 years of age, accounting for approximately 80% of cases in this age group. Given the demographic aging in Portugal, it is imperative to empower these clients for a safe transition, utilizing Dorothea Orem's theoretical framework. Objective: To identify nursing care interventions that promote the empowerment of the elderly with heart failure during a safe transition to the home. Methods: An integrative literature review (ILR) was conducted based on the Whittemore and Knafl framework. The search was performed in the MEDLINE Ultimate and CINAHL Ultimate databases, including articles published between 2019 and 2024. Results: Three articles were selected. The results highlight the importance of individualized care in improving quality of life and reducing readmissions. It was evidenced that 63% of this population presents cognitive impairment, making the involvement of caregivers in health education essential. The management of polypharmacy was also identified as a central focus of intervention. Conclusion: The promotion of self-care and family involvement are crucial for a safe transition and the reduction of hospitalizations. The nurse plays a fundamental role in empowering the elderly for the self-management of the disease.
