THERAPEUTIC INNOVATIONS IN NECROTIZING FASCIITIS, A LITERATURE REVIEW

Introduction: Fournier's syndrome (Fournier's gangrene) is a polymicrobial necrotizing infection that affects the perineum and can be fatal. Its traditional treatment includes broad-spectrum antibiotic therapy and immediate surgical debridement. The introduction of hyperbaric oxygen therapy (HBO) has been explored as an adjuvant therapy, as it improves oxygenation and inhibits bacterial toxins. Materials and Methods: The research was a systematic review that followed the PRISMA protocol, searching for articles in databases such as PubMed, Science Direct and Lilacs, published in the last five years. Seven studies analyzing the efficacy of OH in combination with standard treatment were selected. Results: Most of the studies reviewed showed that OH was effective in reducing mortality and improving clinical outcomes, with six of the seven studies demonstrating a positive association. Discussion: Hyperbaric oxygen therapy (HA) shows promise in the treatment of Fournier's Syndrome by fighting anaerobic bacteria and promoting healing. However, studies show varying results regarding the efficacy of HA in reducing mortality and improving clinical outcomes. Most of the available research is observational and may contain biases, indicating the need for more rigorous studies, such as randomized clinical trials. Conclusion: OH can be an effective adjuvant therapy for Fournier's Syndrome, potentially reducing mortality and improving clinical outcomes. However, due to the methodological limitations of current studies, clinical trials are needed to confirm these benefits and establish more solid guidelines. If proven effective, access to OH could become relevant in public health.
