Abstract: Dental surgeons are qualified to use drug therapies for various oral pathologies. Antibiotic therapy in pediatric patients is common and represents a significant portion of dental prescriptions in Brazil. However, the indication of these medications requires extensive knowledge, both due to potential drug interactions and the risks associated with microbial resistance—especially in children, whose body weight and size demand greater dosing precision. The excessive use of antibiotics in non-recommended clinical situations can contribute to the early presence of multidrug-resistant bacteria. This study aims to highlight the main therapeutic indications for antibiotics in pediatric dentistry, focusing on the appropriateness of prescriptions and the promotion of the rational use of these drugs in accordance with current clinical guidelines. Through a qualitative and descriptive literature review, national and international studies were selected that address the prescription of antibiotics in children with primary or mixed dentition. The use of antibiotics is unnecessary in locally controlled situations and should be restricted to specific cases, such as infections with systemic signs, edematous abscesses, general patient compromise, dental trauma with risk of infection, or prophylactic needs. Additionally, it is essential to select the appropriate type of antibiotic, as well as the correct dose and duration of use, avoiding empirical and generalized prescriptions. Therefore, the study reinforces the need for professionals to follow updated and well-defined protocols, prioritizing conservative clinical approaches in order to ensure pediatric patient safety and treatment effectiveness.
