Abstract: Introduction: Climacteric represents a transitional stage from the fertile to the non-fertile period of a woman's life, marked by hormonal, physical, and emotional changes. During this period, there is an increased risk of chronic diseases, as well as significant impacts on women's self-esteem and autonomy. In societies that value youth and thinness, such as Brazil, these transformations tend to intensify body dissatisfaction and reduce self-care during climacteric. Objective: To analyze the self-perception of body image and adherence to strength training in climacteric women. Method: This is a cross-sectional, quantitative study, approved by the Research Ethics Committee (opinion no. 6.808.315). One hundred climacteric women, aged 40 or older, residing in the southern zone of São Paulo, were interviewed. Data collection was conducted through individual face-to-face interviews using a tablet with an electronic form, composed of three sections: self-perception of body image, physical activity practice, and anthropometric measurements. Results: The majority of participants (86%) recognized their body image as overweight or obese. Of the total sample, 94% presented some degree of body dissatisfaction. Furthermore, almost half of the women (45%) reported a desire for overweight silhouettes, indicating possible perceptual distortion and normalization of excess weight. Regarding physical activity, low adherence to strength training was observed (35%), with only a quarter of the studied group practicing it regularly (≥3x/week). Conclusion: The evaluated sample presented high body dissatisfaction and low adherence to Strength Training (ST), reinforcing the need for multidisciplinary strategies that integrate physical and mental health, encourage the practice of ST, and promote a more positive relationship with body image, contributing to active and healthy aging for climacteric women.
