Abstract: The Grynfelt triangle, also known as the superior lumbar triangle, is defined superiorly by the 12th rib, laterally by the internal oblique muscle, and medially by the quadratus lumborum muscle. In the face of the discontinuity of the posterolateral abdominal wall, there is extrusion of intraperitoneal and extraperitoneal organs, resulting in Grynfelt hernia. This article portrays the case of a female patient who presented with abdominal discomfort unresponsive to clinical treatment and worsen symptoms with moderate intensity exercise, associated with the emergence of a progressively growth bulging in the right subcostal topography. After investigation, the patient was diagnosed with Grynfelt hernia, and surgical repair with internal oblique muscle raffia and Onlay mesh placement was indicated. Lumbar hernias account for about 1.5% of abdominal hernias. Their presentation is rare, and there are few reports on this topic in the literature, emphasizing the importance of proper diagnosis and knowledge regarding the best approach.
