Abstract: This article aims to present the case report of a young male patient, hospitalized in a psychiatric ward with prodromal psychotic symptoms, based on Klaus Conrad’s theory. The study refers to a patient hospitalized in the male ward of the psychiatry unit at the Hospital Universitário São Francisco, in the city of Bragança Paulista – SP, whose clinical presentation corresponds to the model of prodromal symptomatology described by Conrad in Die Beginnende Schizophrenie (1958) This symptomatology is organized into stages, Trema, Apophany, Anastrophe, Consolidation and Residue, in which an intense sense of strangeness toward the world and inexplicable fear is observed, possibly evolving into psychotic symptoms such as delusions, hallucinations, affective and behavioral changes. Although its identification is uncommon in clinical practice, recognizing these stages is of great value for the early detection and treatment of psychotic disorders, favoring a better prognosis. In the reported case, the review of the patient's clinical evolution, carried out through the analysis of his medical record and a discussion based on the literature, evidenced the presence of the initial symptoms and a progression compatible with these stages, with the progression of the condition being, fortunately, positive.
