Abstract: Alopecias comprise a heterogeneous group of disorders characterized by partial or complete hair loss, significantly affecting patients’ quality of life. In recent years, regenerative medicine has gained prominence in dermatology due to the development of innovative therapeutic approaches focused on tissue repair and biological modulation of hair follicles. In this context, exosomes have emerged as one of the most promising technologies in modern trichology. These extracellular vesicles mediate intercellular communication and transport proteins, lipids, growth factors, cytokines, and microRNAs capable of directly influencing hair growth mechanisms. This study aimed to review current scientific evidence regarding the use of exosomes in the treatment of alopecias, addressing their mechanisms of action, clinical applications, benefits, limitations, and future perspectives. A narrative literature review was conducted using PubMed, Scopus, and Web of Science databases, including publications from 2019 to 2026. The analyzed studies demonstrated that exosomes may stimulate dermal papilla cell proliferation, promote angiogenesis, modulate inflammatory processes, and prolong the anagen phase of the hair cycle. Despite promising results, challenges remain regarding protocol standardization, regulation, and long-term efficacy. Exosomes represent an important frontier in regenerative medicine applied to trichology and may transform the management of alopecias in the coming years.
