Abstract: Acute fatty liver of pregnancy (AFLP) is a rare obstetric emergency with significant maternal and fetal morbidity and mortality. This study reviews literature from 1987 to 2025 across SciELO, PubMed, Google Scholar, and Capes, focusing on epidemiology, pathophysiology, clinical presentation, differential diagnosis, management, prognosis, and implications for future pregnancies. Emphasis is placed on fetal fatty acid oxidation defects, the utility of Swansea criteria, and the necessity of immediate obstetric intervention. Despite advances in obstetric intensive care, AFLP demands high clinical suspicion and multidisciplinary management.
