Abstract: The study aimed to critically evaluate the efficacy and safety of intranasal ketamine compared to oral antidepressants in the treatment of adults with treatment-resistant depression. This is a systematic review conducted based on the PICO strategy, using the PubMed database and including clinical trials and randomized controlled studies published between 2021 and 2026. A total of 10 studies were selected after applying rigorous inclusion and exclusion criteria, addressing outcomes such as symptom remission, time to therapeutic response, and safety profile. The results demonstrated that intranasal ketamine produces a significant reduction in depressive symptoms, with a faster onset of action and higher remission rates compared to oral antidepressants, including superiority over quetiapine in phase III studies. Additionally, evidence showed maintenance of remission in the long term, no relevant cognitive or olfactory impairment, and a consistent safety profile, even with prolonged use or in combination with other medications. It is concluded that intranasal ketamine represents an effective and safe therapeutic alternative for patients with treatment-resistant depression, standing out for its rapid action and improved clinical outcomes.
