Abstract: Rotator cuff injuries represent one of the main causes of shoulder pain and functional disability, especially in individuals over 40 years of age. Surgical treatment can be performed through open surgery or arthroscopic repair, both of which are associated with postoperative physiotherapeutic rehabilitation. The objective of this systematic review was to compare functional, clinical and rehabilitation outcomes between open surgery and arthroscopic repair in the treatment of rotator cuff injuries, emphasizing the role of physical therapy in functional recovery. A systematic review was carried out based on PRISMA recommendations, using the PubMed, Medline, Scopus and Cochrane Library databases. The PICO strategy included patients with rotator cuff injuries undergoing open or arthroscopic surgery, comparing functional outcomes and physical therapy rehabilitation. Clinical trials, prospective studies and systematic reviews published between 2023 and 2026 were included. The studies demonstrated that arthroscopic repair presents less tissue aggression, less postoperative pain and earlier functional recovery when associated with structured physiotherapeutic protocols. Open surgery remains indicated for extensive and irreparable injuries, presenting satisfactory functional results in the long term. Physiotherapy proved to be essential in both surgical methods, contributing to gaining range of motion, reducing pain, muscle strengthening and functional return. It is concluded that arthroscopic repair associated with physiotherapy has important clinical advantages, although both techniques can produce satisfactory results when accompanied by appropriate rehabilitation protocols.
