Abstract: Advances in congenital heart disease surgeries have significantly increased patient survival. However, the focus has shifted from immediate survival to quality of life and long-term outcomes, which are often impacted by complications and variability in quality of care. This review sought to analyze cardiovascular development and long-term complications in patients undergoing congenital heart disease surgery, as well as explore innovations and strategies to improve care based on 23 selected articles. The results revealed that, although significant advances have been made in the survival of patients with congenital heart disease, it remains lower than that of the general population, directly compromising quality of life, physical growth, and neurological development. In this context, the study also highlights the influence of genetic factors on prognosis and the considerable variability in outcomes between different surgical centers. Innovations such as telemedicine and the use of advanced cardiac devices have shown promise in improving care coordination and improving the quality of care provided. In short, the success of these surgeries must be assessed through a broader spectrum of long-term outcomes. To this end, holistic and multidisciplinary management is essential, integrating genetic advances, technological innovations, and special attention to psychosocial well-being. Standardization of care, regionalization of high-complexity centers, and the use of telemedicine tools are essential to ensure that increased survival is accompanied by a better quality of life.
