Abstract: Hydrocele is a condition characterized by the accumulation of serous fluid between the layers of the tunica vaginalis, which can cause discomfort, scrotal swelling, and impaired quality of life. Objective: to report a clinical case of bilateral testicular hydrocele surgically corrected using the Lord technique. Case description: a 56-year-old male, smoker, with diabetes mellitus and hypertension, presented bilateral scrotal enlargement, discomfort, and progressive erectile dysfunction. Ultrasonography confirmed serous fluid around both testicles. Bilateral hydrocelectomy using the Lord technique was performed with good postoperative evolution and no recurrence after three months. Conclusion: surgical correction was effective in symptom resolution and functional improvement, emphasizing the importance of comorbidity control and regular urological follow-up.
