Abstract: This report describes the case of a 50-year-old female patient who had previously undergone mastectomy and breast reconstruction with a textured prosthesis. She presented with volumetric increase, pain, and inflammatory signs in the right neomamma, which appeared late in the pregnancy. Given the presence of a fluid collection associated with the implant, breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) was suspected, a rare but recognized condition in patients with textured prostheses. However, cytological analysis and histopathological examination ruled out malignancy and revealed a periprosthetic granuloma. This case highlights the diagnostic difficulty arising from the clinical overlap between benign inflammatory processes and BIA-ALCL, emphasizing the scarcity of literature on implant-associated granulomas and the importance of adequate investigation for appropriate management.
