Abstract: Introduction: Pediatric solid tumors remain a major clinical challenge, requiring therapeutic strategies that balance efficacy with long-term safety. Recent advances in targeted therapy and immunotherapy have reshaped the management of these neoplasms, promoting a more personalized approach based on specific molecular mechanisms. Methods: A systematic review was conducted in the PubMed, SciELO, and LILACS databases, covering the period from 2019 to 2025. Original studies, systematic reviews, and clinical trials available in full text addressing these therapies in pediatric solid tumors were included. Data were extracted and descriptively synthesized following PRISMA 2020 recommendations. Results: Twelve studies met the eligibility criteria. Targeted therapies showed significant clinical benefit in subgroups with ALK, NTRK, and BRAF mutations, with favorable safety profiles. Immunotherapy, including monoclonal antibodies, immune checkpoint inhibitors, and CAR-T cell therapies, demonstrated promising activity in refractory tumors, despite limitations related to the tumor microenvironment and pediatric immune variability. Conclusion: Targeted therapy and immunotherapy represent a major step forward in pediatric oncology, offering more effective and less toxic alternatives to conventional treatment. The field’s future relies on the integration of molecular research, multicenter clinical trials, and equitable access to precision oncology innovations.
