Abstract: Ultrasound-guided peripheral nerve block has become the standard approach for regional anesthesia, offering unparalleled safety and precision. This study aims to synthesize recent evidence on the technique's application, exploring scenarios of proven superiority, cases of clinical equivalence with other techniques, and procedural optimization strategies. An integrative analysis of recent clinical studies evaluating peripheral nerve block in various contexts was performed. The investigations compared ultrasound with traditional techniques, different ultrasound approaches to the same target, new ergonomics, and the impact of variables such as anesthetic temperature. Ultrasound guidance demonstrates clear superiority in certain procedures, such as the treatment of chronic migraines and in the selection of a specific approach for laryngeal nerve block, resulting in greater efficacy and reduced complications. However, this superiority is not universal. In other scenarios, such as pericapsular hip block or analgesia for thoracic surgery, ultrasound- guided techniques have been shown to be clinically equivalent to blind methods or those performed under direct vision. Furthermore, the choice of the ideal block can be influenced by factors such as secondary patient symptoms or overall satisfaction, even in the presence of adverse effects such as rebound pain. The future of excellence in peripheral nerve blocks lies not only in mastering ultrasound but in the ability to refine its application. Clinical practice must evolve from a one-size-fits-all approach to a personalized strategy that integrates the best evidence.
