Abstract: Radical prostatectomy (RP) is a common treatment for prostate cancer, but erectile dysfunction (ED) is a frequent complication. This retrospective study evaluated the prevalence and factors associated with ED in 180 patients who underwent RP at a university hospital (January 2017– December 2019), considering age, comorbidities (systemic arterial hypertension - SAH, diabetes mellitus - DM), adjuvant radiotherapy (RT), ISUP grade (from biopsy and surgical specimen), and preoperative PSA. Data were collected from electronic medical records, and ED was assessed for up to three years post- surgery. Statistical analysis used Chi- square, Fisher's exact, and likelihood ratio tests (p≤0.05). The mean age was 64.1±7.3 years. There were no statistically significant associations between ED and age, SAH (p=0.900), DM (p=0.909), adjuvant RT (p=0.060), biopsy ISUP (p=0.429), specimen ISUP (p=0.733), or PSA (p=0.320). A slight trend of higher ISUP in the group with ED and lower ED in the group with adjuvant RT was observed (not significant). It is concluded that, in this cohort of patients operated on by multiple surgeons, no significant clinicalpathological predictors for post- RP ED were identified. Limitations include uncontrolled surgical variability and ED assessment based on medical records. ED remains a significant concern, and surgical technique-related factors are likely crucial, requiring robust prospective studies.
