Abstract: Primary Health Care (PHC) is the foundation of the Brazilian health system and plays a fundamental role in the continuous monitoring of the population, especially in the management of chronic diseases such as hypertension and diabetes mellitus. In this context, teamwork, particularly between physicians and nurses, is essential to ensure comprehensive and continuous care. This study aims to discuss the joint performance of these professionals in PHC and its implications for health care. This is a narrative literature review conducted through the analysis of articles available in the Scientific Electronic Library Online (SciELO) database, as well as official documents and legislation related to the topic. The findings indicate that the integrated performance between physicians and nurses contributes to improving the quality of care, promotes continuity of care, and strengthens the bond with patients. Nurses play an important role in longitudinal follow-up and educational actions, while physicians are responsible for clinical evaluation and therapeutic decision-making in a complementary manner. It is concluded that the integration between these professionals is a fundamental element for the proper functioning of PHC and for the effectiveness of health care delivery.
