Abstract: Population aging in Brazil has increased the demand for a care model able to respond to the clinical and social complexity of older adults within Primary Health Care. This article aimed to discuss, based on a literature review, how Primary Health Care has addressed challenges related to autonomy, chronic disease management, and contemporary barriers in the care of older adults. This is a narrative review based on four national studies published between 2014 and 2021, selected for thematic relevance to the Brazilian context. The findings indicate that Primary Health Care has a strategic role in coordinating care for older adults, especially by promoting longitudinal bonds, continuous follow-up, and preventive actions. However, relevant limitations remain, including barriers to access, fragmentation of the care network, team overload, socioeconomic inequalities, and insufficient integration among health services, families, and communities. It is concluded that strengthening Primary Health Care requires interdisciplinary practices, continuous professional qualification, qualified listening, and reorganization of care focusing on comprehensiveness.
