Abstract: Introduction: Migraine is a frequent and disabling neurological disorder. Its association with cardiovascular diseases has been more extensively investigated in the subtype with aura, while evidence regarding migraine without aura (MWoA) remains less consistent. Objective: To evaluate the relationship between migraine without aura and the risk of developing cardiovascular diseases. Methods: A systematic review was conducted in the PubMed/MEDLINE database, including articles published between 2020 and 2025, in Portuguese or English, with full-text availability. Following PRISMA 2020 guidelines, 7 studies were selected for qualitative synthesis. Results: MWoA was associated with an increased risk of ischemic stroke, systemic arterial hypertension, and ischemic heart disease. The cardiovascular risk was particularly evident in young women, smokers, and oral contraceptive users. Proposed pathophysiological mechanisms include endothelial dysfunction, hypercoagulability, oxidative stress, and genetic predisposition. Conclusion: Migraine without aura may represent a clinical marker of cardiovascular risk, although current evidence is limited by methodological heterogeneity. Further studies are required to deepen the understanding of underlying mechanisms and to define preventive and monitoring strategies.
