Abstract: Introduction: Zenker’s diverticulum is a sac-like projection of the mucosa at the pharyngoesophageal junction, predominantly affecting elderly patients. It can cause limiting symptoms such as halitosis, regurgitation, dysphonia, and significant dysphagia. Treatment is primarily surgical, with endoscopic approaches including the conventional technique and Z-POEM.Objectives: To compare the outcomes of conventional endoscopic management versus the Z-POEM technique in patients diagnosed with Zenker’s diverticulum at a tertiary hospital in Curitiba, in order to determine which technique provides better results in reducing dysphagia severity and lower recurrence rates after 6 months.Methods: Retrospective and prospective cohort study based on the analysis of surgical outcomes collected from electronic medical records at a tertiary hospital in Curitiba, between January 2020 and August 2024. Outcomes included regression of dysphagia severity (measured by the Dakka and Bennett scale) and recurrences within six months after surgery in 23 patients operated by the same surgeon. Of these, 11 underwent the conventional endoscopic technique and 12 underwent Z-POEM. The association between the two surgical techniques as categorical variables was tested using the Chi-square test, and dichotomous variables with Fisher’s exact test. Data were analyzed using IBM SPSS Statistics v.28.0 (Armonk, NY: IBM Corp.), with p < 0.05 indicating statistical significance.Results: The groups were similar regarding age (Z-POEM: 78.07 ± 7.16 years; Conventional: 73.73 ± 7.64 years; p=0.12), sex (Z-POEM: 50.0% female; Conventional: 27.3%; p=0.49), obesity (Z-POEM 33.3%; Conventional 27.3%; p=1.00), hypertension (Z-POEM 50.0%; Conventional 81.8%; p=0.19), diabetes (Z-POEM 0.0%; Conventional 9.1%; p=0.47), and smoking (Z-POEM 16.7%; Conventional 18.2%; p=1.00). Mean dysphagia reduction was greater in the Z-POEM group (2.80 ± 0.41 points) compared to the Conventional group (2.07 ± 0.59 points; p=0.02), with 75.0% and 27.3% of patients, respectively, reaching grade 0 (p=0.03). Recurrence rates were 25% in Z-POEM and 27.3% in the Conventional group (p=1.00).Conclusion: Z-POEM proved superior in terms of symptom reduction, as demonstrated by the greater mean reduction in dysphagia severity and the higher percentage of patients achieving complete symptom regression (Grade 0). Furthermore, Z-POEM resulted in a lower recurrence rate compared to the conventional technique, although this difference did not reach statistical significance.
