Abstract: Prescription errors can occur both during the drafting of the prescription and throughout the decision-making process regarding treatment. Studying this phenomenon in Brazilian hospital settings is essential to strengthening patient safety and improving the quality of healthcare delivery. In this context, the present study aims to investigate how prescription errors impact the health of the Brazilian population. This work consists of a literature review based on publications indexed in Google Scholar, SciELO, and PubMed, using the descriptors “medical prescriptions” AND “prescription errors.” Prescription errors represent a significant category within medication errors and may arise from the writing phase of the prescription to the clinical decision-making process. Among the main triggers are insufficient information about the patient or provided by the professional, incomplete prescriptions, and illegible handwriting. It is estimated that the rate of prescription errors is 8.9 errors per 100 medical prescriptions, while hospitals present an incidence of approximately 4.9%. Ensuring adequate prescription practices is essential to promoting the Rational Use of Medicines, requiring strict compliance with current legislation and clinical best practices. The direct involvement of pharmacists with patients and multidisciplinary teams plays a strategic role in preventing and resolving medication-related problems in a systematic and well-documented manner. Furthermore, the implementation of electronic prescribing systems represents a significant advancement, contributing to the reduction of failures associated with manual prescriptions and enhancing the safety of the therapeutic process.
